MixWizard WZ1 Troubleshooting

My MixWizard 14:4:2 is emitting a pulsing clicking sound on every channel. Any suggestions for troubleshooting or ideas about what might cause this? See video link below.

Is the phantom power turned on?
Does it still pass audio even with the clicking?

I guessing there is a power supply issue.

Phantom power is off. No, it’s not passing any audio. I bought it about a month ago on Reverb for routing/bussing flexibility while tracking with outboard gear. The first time I tested it, I didn’t notice any issues. The only thing I’ve done since then is connect the line ins and buss outs to my patchbay. As you can see in the video, the problem is present with all lines disconnected.
