Just to the left of the Headphone Jack, there is a “PFL/AFL Wedge Monitor” button,
I suspect that has been pressed.

You can process 48 channels at any given time, how ever you can change between any of up to 128 physical inputs between scenes (just recall different preamps in the I/O )
You have removed it from the Layer in Settings/Surface.
As Steffen writes, to me it sounds like you did not patch the RiO to the SQ6 in Dante Controller ?
First thing to my mind is: Mic Ext on the Main bus.
Just 1 Mute Group would be stellar, but 2 or 3 will suffice 😉

SQ Dante – 64channels
SQ Waves – not 100% but think it is 64 as well.
For Dante and recording, you can look at DVS (Dante Virtual Soundcard), it gives you 64 channels at 48KHz or 32 channels at 96KHz (the Dante card has a samplerate converter build in )
Waves I have no experience with recording.

You have selected a DCA, normal behavior.
The DCA’s have no processing (and thus no EQ), only the ability to combine fader/mute levels on the channels placed into each of them.
(DCA is the digital version of the analog consoles VCA)
Just wanted to make sure, as I have seen people copy the downloaded .zip file onto the USB key without unzipping it.
A few other things that “might” be connected:
The USB key is formatted on the console ?
And naturally you have the correct console type of update (SQ5, SQ6, SQ7)
Just to be sure, you did unzip the file you downloaded from the site?
Had a few who missed that step, as windows have a tendency to hide the .zip unless instructed to show filetype.
You need to send the FX return to each Aux as well, just like the IP channels.
One way could be:
Configure the console with 6 mono Aux sends.
Have all inputs set at 0dB on all 6 sends (all set to POST).
Then you have the Aux masters for those sends as “volume” on each monitor, yet if needed you can still change the levels of each channel send to each monitor.
Alternative is to configure 6 mono Groups instead of AUX, and assign all channels to all Groups.
The DM3 has a 18×18 USB interface on the back beside the normal 2track stick in the front.
I feel a SQ3 with a similar footprint and perhaps half channelcount as SQ5/6/7 could be a killer for smaller venues/meeting rooms etc.
